One line per decision, with who decided and a link to the discussion. When scope is questioned two weeks later, the answer is a link instead of an argument.
Tie decisions to projects
A decision to launch on a given date should create the tasks that depend on it: the flow, the API work, the screens, the beta cohort. Each should name an owner and carry the date.
Review the open commitments weekly
Look at what is open, what is overdue and what has no owner. That list replaces most status meetings.
